
At College Health Practices, we support children and young people with Special Educational Needs and/or Disabilities (SEND).
We work with parents and carers, schools, and Thurrock Local Authority to keep children healthy, up to date with immunisations, and connected to the right services at the right time.
We aim to wrap care around the needs of each young person with SENDs.

How we can help:
- Allocation of a Named GP and/or Nurse to ensure continuity of care.
- Longer/quiet appointments and reasonable adjustments for children who find health visits hard.
- Early identification of developmental, communication, physical or emotional needs and referral to the right services.
- Care coordination with community paediatrics, therapies and school health teams.
- Health information to support Education, Health and Care Plans (EHCPs).
- Vaccination support, including catch-up plans and preparation tips

Immunisations & Keeping Well
Keeping up to date with vaccinations is one of the best ways to protect your child. If your child finds needles or busy clinics difficult, let us know. We can offer longer appointments, a quieter space, or other reasonable adjustments. Please contact the practice if your child is missing any vaccinations.


Need Support or Have Questions?
Speak to your GP or Nursing team at your College Health Practice. With your consent, we can liaise with your child’s school SENCO, school nurse and the Local Authority SEND team to make sure support is joined up.
